Out-of-State Patient Registration Process
Step 1: We must receive a brief letter, fax, or email from your provider (e.g. primary care physician, psychologist, psychiatrist, therapist, sleep specialist, or other provider) simply stating that you are being referred to our center to assess or reassess your current sleep treatment regimen.

Step 2: Complete our online intake system. This will take you approx: 30 to 60 minutes.

Step 3: We will need copies of all pertinent sleep medical records including sleep tests and clinic notes. We prefer these be faxed or emailed.

Out-of-State Patient Treatment Process
Once the 3 registration steps have been completed, Dr. Krakow will review your intake and your past sleep medical records.
- You'll be scheduled for a one-hour Skype consultation with Dr. Krakow.
- Dr. Krakow will prepare an extensive report making recommendations to improve your sleep treatment regimen.
At this point in the process additional options may be available, including:
- We will provide Dr. Krakow's report to your current sleep physician or provider.
- We may arrange further dialogue between Dr. Krakow and your physician or other provider, as needed.
- In some cases, patients may elect to visit our center here in Albuquerque, NM.
Billing: All payments are the responsibility of the patient receiving services, and all fees for treatment services must be paid at or before the date of service. If a credit card is used, then payment can be made on the day of the consultation at the completion of the appointment. If a check or money order is the method of payment, it must be received by the Center prior to the date of the consultation.
Insurance: No direct payments or referrals through health insurance carriers will be accepted by the Center; however, you will be provided with physician notes that contain full documentation of your evaluation and treatment plans. In some instances, you may be able to use these documents to obtain reimbursement from your health insurance plan. You are welcome to pursue coverage for services provided at the Center through traditional means of primary care referrals, preauthorizations and so forth; however, there is no need to inform us of this intention as all medical notes will be provided directly to you in a timely fashion, usually within 2 working days after completion of any appointment. As a reminder, all payment for services must be provided directly from the patient or a responsible party representing the patient at or before the appointment.
Last, if your recommendations include additional testing in a sleep lab and you choose to visit our center, you need to know that we take all major insurance carriers, so it is probable (but not guaranteed) that your insurance will cover your testing and appointments at our center in New Mexico. As a courtesy to you, we would make every effort to determine your coverage before you schedule a trip to visit us.
