About the Online Questionnaire |

The online questionnaire is designed to help you initiate contact with the sleep center in a time-saving way. By completing this confidential series of questionnaires, you will create a SLEEP PROFILE, and we can make a preliminary assessment of your sleep problems. After you submit your SLEEP PROFILE, we'll call or email you, usually within 48 hours, to discuss how to begin your evaluation and treatment at our sleep center. The questions asked about your sleep are very interesting and thought-provoking. Most sleep patients tell us they learned valuable information just by completing the questionnaires.
